💳 The laboratory bills your insurance directly for testing services

👉 Any deductibles, copays, or non-covered services remain the patient’s responsibility based on their individual insurance plan

✔ Test results are released by the laboratory to the ordering healthcare provider

 

 

💻 Option 2: Physician Requisition + Self-Pay (No Insurance)

🏥 Specimens are delivered to the laboratory designated on your requisition
💳 The laboratory bills you directly for testing and processing

All testing services, billing, and result reporting are handled solely by the laboratory identified on the physician requisition.
